Access Team
The Community Care Access Team makes it easier for people to
access Community Care services. If you think you require support
from Social Work, Occupational Therapy, Care at Home, Community
Learning Disabilities Team or Drugs and Alcohol Services then you
can contact the Community Care Access Team.
The team is the first point of contact for anyone aged 16 or
over requires support services. The team is available from
Monday to Friday, between 8.45am and 5pm. Outwith these
times, contacting the team's number will connect them to the
out-of-hours service.
If you or someone you know needs assistance you can contact the
Access Team to discuss this. We can offer advice and information on
a range of services.
For example we can offer advice on the following-
- Support to carers including advice, respite breaks.
- Care at home services including provision of meals, community
alarm.
- Access to residential care, or services for learning
disability, mental health or addiction.
- Welfare rights / help with benefits.
- Occupational Therapy service to help you remain independent
through the provision of equipment and adaptations.
- Referral for Social Work support.
If you do require support from Community Care Services you may
require an assessment, which will involve an appropriately
qualified person meeting with you to help you identify what support
you require to help you reach your potential or assist you with
every day activities.
If you are in crisis, or you are calling on behalf of someone
who is in crisis, the Access Team will provide immediate
assistance.
You may see and hear things that make you worry about an adult's
welfare and safety. It is everyone's responsibility to ensure
people are not being harmed. To report any concerns, contact the
Access Team, and someone will be able to assist 24 hours a day.
People can call into any Council building and be put in touch
with the team. The team itself is based in Pullar House in
Perth.
If you already receive a service, you should get in touch with
your usual contact(s), and they will deal with your enquiry.
