Evidence Required for Companion Pass
The applicant must produce the following
documentation for verification:
1. The applicant is in receipt or has been awarded
the higher or middle rate of the care
component of Disability Living Allowance (DLA).
Evidence - the original award letter
the Department for Work and Pensions (DWP) or confirmation of award
on form DBD 384 stating that the applicant is in receipt or has
been awarded the higher or middle rate of the care component of
DLA. The award date should not have expired and the evidence should
be dated within the last 12 months.
2. The applicant is in receipt or has been awarded
Attendance Allowance issued by the DWP.
Evidence - Attendance Allowance
award notice or letter from the DWP stating that the applicant has
qualified for Attendance Allowance. The award date should not have
expired and the evidence should be dated within the last 12
months.
3. The applicant has been certified as blind and in consequence
is registered as blind in a register maintained by or on behalf of
a council constituted under the Local Government (Scotland) Act
1994.
Evidence ? letter from Local
Authority, RNIB, or other Blind Society confirming the applicant is
registered as blind on a register maintained by or on behalf of
their local authority.
4. The applicant lives in a care/residential home or hospital
and who are eligible for the higher or middle rate of the care
component of the DLA or Attendance Allowance.
Evidence - An original award letter DWP or
confirmation of award on form DBD 384 stating that the applicant is
in receipt of higher or middle rate of the care component of DLA.
An original award notice from DWP or confirmation of award on form
DBD384 stating that the applicant is in receipt of Attendance
Allowance. The award date should not have expired and the evidence
should be dated within the last 12 months.
