Older People Aged 60 and Over - New Applicants

Local Post Offices will process all Entitlement Card applications from Older People.

The Entitlement Card application form (LS304) to be used by new applicants who are aged 60 and over is available from Post Offices in Perth and Kinross.  

Copies of Form LS304 are also available from Council offices.  Alternatively you should contact the Council's Public Transport Unit (telephone 0845 603 0976 or e-mail concessionarytravel@pkc.gov.uk) - and they will post out an application form to you.  

You can apply for an Entitlement Card two weeks in advance of your 60th birthday - but your Entitlement Card will not be activated until the date of your birthday.  

You must be a resident of Scotland when applying for an Entitlement Card and you will be required to provide one form of evidence for verification purposes. Your name and a Scottish residential address must be clearly stated on the document supplied. Click here for a list of suitable documents that can be used for proof of your residency.

You must also produce one of the following as evidence of being aged 60 or over:
  • Birth Certificate Current
  • UK Driving Licence Current UK Passport
  • Attendance Allowance award letter from DWP stating that you have qualified for Attendance Allowance

Please note that Transport Scotland have indicated that a UK driving licence cannot be used to prove residence and age, two separate forms of identification will be required.  

Completing the Application Form  

You must complete Parts 1, 2 and 3 of Form LS304.  Parts 4 and 5 are for official use only and should not be completed by you.  

You should attach a passport-type photograph to the form, which should be in colour, and have your name and form number (top right-hand corner of Page 3 under the bar code) written on the reverse in Block Letters.
 
The photograph which is attached to the form should be recent (taken within the last year), be you showing a close-up of your head and shoulders so that your face covers approximately 65 to 75% of the photograph and your features should be clearly distinguishable against the background.  The photograph should be in sharp focus and printed on plain white photo quality paper.

You should take the completed application form and photograph, together with the required proof of age and address, to any Post Office in Perth and Kinross.

The member of staff in the Post Office will check that Parts 1, 2 and 3 of the form have been completed correctly and that the photograph is a true likeness of you.  They will also check your proof of address and age, returning the evidence documents to you.

The Post Office will then forward your application form to the organisation that is responsible for issuing the Entitlement Cards.  Your Entitlement Card will not issued directly by the Post Office.  

It may take up to ten working days for your application form to be processed and for you to be sent your Entitlement Card to your home address.

Transport Scotland has advised Perth & Kinross Council that applicants must not post their application forms directly back to the Council.  Completed applications must be presented at a local Post Office.
