Step up care

Step up care is an alternative support for someone who is unable to safely remain in their own home just now, but long-term residential care or a hospital admission is not immediately necessary.

Service users

The service is for adults over the age of 65 where a GP or social work professional has identified that the current support measures which allow them to stay at home are not enough to remain there safely. The aim of the service is to ensure people can return home, with the necessary support, as soon as it is safe to do so.

Care homes

Step Up Care placements are available at a number of care homes across Perth and Kinross. The decision to give somebody a step up care place at a care home will be discussed with them ahead of time. An important part of this discussion will be whether there are any suitable places available at homes near them or their family.

Costs

There is no charge to anyone who gets a place in a care home through step up care.

Time in step up care

Step Up Care is only intended to be a short term solution to allow alternative support measures to be put in place for the return home. Within 72 hours of admission to the home a meeting will be held involving the individual, their carers/family and relevant professionals to look at what is needed to allow the individual to return home safely.

Outside of office hours (Monday-Friday 8.45 am to 5.00 pm) this number will be redirected to the Out of Hours Social Work Team who can also arrange step up care if required.
