Support for carers in the community
Information about the various supports which can be found in the community to help you in your caring role.
Adult Carer Support Plans
The Council has a duty to provide you with an opportunity to speak to someone about what help would be useful to your caring role. A 'Carers Support Plan' will be written outlining the support which may help you to remain in your caring role.
To arrange an appointment, call the Early Intervention and Prevention service on 0345 30 111 20 or email AccessTeam@pkc.gov.uk.
Emergency Carer's Card
This is a free service for Carers. If you are the carer of someone who would be at risk if something were to happen to you or the parent of a child with complex additional support needs, the Carer's Card could give you the peace of mind and reassurance that the person who depends on you will be looked after in an emergency situation.
If you wish to register, please complete an
Emergency Carer's Card application form [302Kb] and sent to:

Telecare, also known as community alarms, provides peace of mind for individuals and their families through a 24-hour response service. More information is available on our Telecare service page.
