Moving from children's to adult learning disability services
A child or young person with Additional Support Needs (ASN) / Learning Disabilities (LD) or Autism who has been formally assessed as requiring additional support will be eligible for this Transitions Service.
Transitions Services support young people with these additional needs to make the best decisions about future choices such as college, employment and housing.
This may include guidance on money matters, job hunting, healthcare or support at home.
