Reablement

Reablement helps people to regain or develop independence, enabling them to undertake tasks around the home.

Reablement (PDF, 616 KB) is available across Perth and Kinross to help people maintain and improve their independence in the community. 

In Perth and Kinross we are working in partnership with individuals to agree what they want to achieve, and how we can help them do it. The key to Reablement is giving individuals the support and confidence to carry out everyday tasks for themselves. It could mean re-learning how to do the task after a period of illness or an accident, or using small aids to do a task in a new way which takes advantage of their strengths.

Reablement works towards a time where the service is no longer needed by that individual, although that it may be that they continue to receive support from occupational therapy, mainstream home care or telecare to enable them to maintain their independence.

Referrals for Reablement

Referrals will often be through a GP or hospital staff preparing to discharge a former patient back to their home.

However, if you are not already in contact with community care services and would like an assessment of your needs to see if reablement - or any other type of support - is suitable for you, please contact the Early Intervention and Prevention Service on 0345 30 111 20 or email AccessTeam@pkc.gov.uk.

Costs

There are currently no costs for Reablement services itself, however, it may be that as part of the outcomes you are offered additional services such as mainstream home care or a community alarm, for which there are charges. However, these will be fully discussed with you at the time.
