Who are we and what do we do?
The Throughcare and Aftercare Team is the team that will support you to prepare for leaving care and will continue to support you when you have left care and are becoming more independent.
The team will support you in a range of ways, dependent on what your plans are, up until you are 26 years of age.
We are a small specialised team with experience of working with young people in various stages of leaving or having left the care system. We work closely with a range of services and providers including social work, carers, youth workers, education, housing, health and others who may have been or could be involved in supporting you.
If you are looked after and accommodated at age 15 years, we will be in contact with you and your Social Worker to discuss your case being transferred over to the Throughcare team where we will work with you to create a plan to help you to work toward greater independence in all areas of your life.
